Life raft painter not connected to the proper hook during a port state inspection, the inspector found the painter was not connected to the weak hook properly. The pointer was connected to the wrong hook, this life raft was sent for the shore service two months before.

The senior officer has to cross check the jobs carried out by junior also cross check all the jobs carried out by shore staffs.
