Incident:
Ship engine was Mitsubishi during departure, engine got over speed and when tried to stop engine by stopping control room telegraph, fuel racks of the engine did not go to Zero, then tried by emergency stop, still engine didn’t stop. Then finally from locally removed the linkage from the governor to fuel pipe removed pushed the fuel rack to zero then stopped the engine.
