Summary:
Bow Thruster Failure
During departure port , bow thrusters found not starting during inspection bow thruster flexible coupling found broken into pieces. Alignment of the motor checked & Radial miss Alignment with bit frames to be about 2.0 mm where as max allowable misalignment is 0.5 mm, 2 years back bow thrusters seal was renewed by shore workshop after lifting bit motor. It is suspected that when motor was fitted back alignment was not properly checked or corrected.
Next port shore workshop attended vessel to correct BT motor alignment and fitting of new flexible coupling

Corrective Action/ Lessons to be learnt:
When shore workshop is carrying out main maintenance / repair jobs, close monitoring need to be done by ship staff reg alignment tightening torque etc.,

Incident:
Bow thrusters remained not operational for one voyage due to breaking of flexible coupling between BT & Dive motor.
