On one of the tankers where I served, after double banking at Vizag, we were sailing for Mumbai. On the way we heard some ringing noise coming from the engine. Informed the C/E/O and together checked thoroughly and noticed noise coming near #3 & #4 units.

Found No 4 unit piston cooling outlet temperature to be high by 4deg compared to other units.

Opened up both inlet and outlet standpipes and found outlet standpipe nozzle (threaded one) missing. As we were reaching port next day, we replaced it with same one and boxed back. Started the engine and sailed to Mumbai, with little high temperature of the affected unit. After reaching Mumbai we changed the piston completely.

On dismantling the removed piston we found nozzle had damaged the piston skirt by making a 2" hole.
