AUXILIARY ENGINE BREAKDOWN – SAFETY DEVICES NOT FUNCTIONING :

The vessel was a 60,000 DWT bulk carrier. No.3 Auxiliary engine was running. Due to the requirement of more load, the 4/E started No.2 auxiliary engine and went to control room and paralleled No.2 with No.3 generator. By the time the generator’s started sharing the load, the lub oil pipe of No.2 burst out, spilling oil all over. The lub oil low pressure trip did not actuate. The oiler rushed to control room to inform about this to 4/E, but by the time the c/shat seized and engine had to stop and No. 3 tripped on over load, causing black out.

Lesson learnt :

It is imperative that all the safety checks should be religiously carried out on a weekly basis.
