NON RESTORATION OF A VALVE IN THE FUEL LINE AFTER WORK :
It was after the gulf war in 1992. The ship went to Kuwait for loading HSD. The Iraqis had laid the mines in the entry area and only 1 km space was cleared off the mines, to enable the ships to enter and leave the port. Just after sailing out, clear off the break waters, the main engines failed. Tried to start the engine, but attempts failed. All the engine room staff started locating the possible fault, the remaining people were employed on the deck with fire hoses to move the fire mines away if cited. The ship was drifting and everybody became panic. After about 20 mins of tracing the line, it was found that one of the valves in the return line had not been opened after cleaning the filter. Same opened, lines purged and engines started, everything went safely.
Lessons learnt :
after finishing any job it is better to check thoroughly by the person who has not worked in that system as counter check to avoid such happenings.
