DEBRIEFING DURING ISM AUDIT HELD ON BOARD :
1)River pilot although excellent for his knowledge of river, proved to be poor at ship handling, possibly due to over confidence and refusal to listen to Master’s repeated cautionary advise. Insufficient / nil experience at sea, would have contributed to this. Master, therefore, requested not to allocate the pilot again for the vessel during inward pilotage as it involved berthing at approach jetty.
2) If berthing at night, search light from Bridge and ISPS torch from focs’le to be focused on approach jetty to verify the state of jetty.
3)Incase vessel coming alongside to fast, starboard anchor to be definitely let go in order to bring vessel alongside gently.
(The above incident was narrated by a Master who has been regularly calling Haldia every 10 to 14 days for the last 7 years. He had been in command as well as a pilot in Indian ports.)
